I was slowly backing up last night and scraped my wheel against the curb. Along with some nasty curb rash, I noticed this on the sidewall. Is this purely cosmetic or should I replace the tire?

As long as the tire sidewall hasn't been compromised as shipo noted (bubble) and there are no cords showing through the gouge, you should be in good shape. Certainly wouldn't hurt to have a tire professional take a look at it for extra reassurance.

Here's a link with some additional tire safety tips for anyone interested.

I don't mean to resurrect this thread, but I'm assuming you're still driving on the tire. I've taken huge slits to tires and still ran with them. A little rubber tire cement to bandage it back together and its good to go. Like the others said, if you can't see cords, there's no bubble, and the tire isn't leaking from the sidewall, you should be good to go.
